At Connected Path Recovery, we provide comprehensive substance use and mental health assessments for individuals referred by courts, attorneys, employers, treatment centers, or family members. These evaluations are designed to assess risk, identify treatment needs, and provide clear, written recommendations based on professional tools and clinical interviews.

Who Our Assessments Are For 

We provide professional, third-party assessments—not just referrals into our programs. 

  • Individuals involved in legal, family court, or CPS proceedings 

  • Probation, parole, or EAP referrals 

  • Attorneys or judges requesting professional evaluations 

  • Treatment centers or medical providers seeking external input 

  • Individuals or families unsure what level of care is needed 

What the Evaluation Includes 

✅ A clinical interview with a licensed provider using the ASI (Addiction Severity Index) 

✅ A standardized screening tool (e.g., SASSI, AUDIT, DAST, or similar) 

✅ A 3–4 page written report including clinical impressions and treatment recommendations 

✅ Optional consultation call with attorney, case manager, or referring party (with consent) 

✅ Fast turnaround time (typically within 3 business days) 

Optional Add-Ons

  • Court-ordered urine drug screen (by request or referral) 

  • Mental health screening 

  • Trauma or diagnostic inventory (as appropriate) 

  • Referral list for recommended programs (including those outside Connected Path) 

Cost & Insurance

Assessment services are typically private pay and may not be billable through insurance. 

 

We can provide invoices or documentation for reimbursement or legal submission upon request. 

What to Expect

The process is simple and respectful: 

  1. Schedule the appointment via phone or form 

  2. Attend a 60–90 minute session with a licensed clinician (in person or virtual) 

  3. Receive your written report within 3 business days 

  4. Optional follow-up consultation with referring party
