1 / Referral Received — Fill out our quick referral form below, email or fax the referral, or call admissions. Admissions will contact the client within 24 business hours of receipt.
2 / Client Assessment & Placement — We evaluate and recommend the appropriate outpatient track.
3 / Care Team Collaboration — With client consent, we keep you updated with regular progress reports.
4 / Discharge Planning — You receive a detailed discharge summary & continuingcare plan.
